Human in the loop means a named person reviews, can override and remains accountable for a decision that an automated system informs. The system supplies measurement. The person supplies judgment and signs the decision.
Automation is useful where a task is repeatable and checkable, such as scoring a file the same way every time. It is not a replacement for the decision to commit capital, accept a risk or settle a matter, which depends on mandate, portfolio context and information the model never saw.
A well-designed pipeline records where the human acted: what the system recommended, what the person decided and why. That record is part of the decision record and is used to review both the model and the decision-maker.
Human review is not a way to launder a weak model. The model still has to pass its gates, and the person is expected to challenge outputs, not ratify them.
